This can include personal care and rehabilitation support, meal preparation, help with shopping and chores in the home, support with taking medication, companionship, help to get out and about to socialise or to attend hospital or G.P. appointments.

Our care is person-centred and can be provided to people of all ages, disabilities and health conditions, 24 hours daily with access to emergency support.

Our services can be accessed in a variety of ways. We are able to advise you if you are self-funding or eligible for statutory funding to cover the costs of your care. Please do not hesitate to contact us for advice if you are in need of a support care package for your family member.

Funding your care in a variety of ways

There are many ways of funding your care. Funding can be through; Continuing Health Care, Social Care, a Personal Budget or a Personal Health Budget, or independently through your own finances.

We are also able to provide health care support for people living at home with chronic health or palliative care needs, funded by the Local Authority. The referral pathway is usually via Hospital Discharge Teams, Community staff and Health Professionals.

End of Life Care: Connected Palliative Care

Crossroads is a member of the award-winning Connected Palliative Care team – a specialist end of life care service in Sandwell. The team can be contacted via the Connected Palliative Care Hub on 0121 507 3611, between 8 am – 8 pm. Out of these hours, for urgent telephone advice, they can be contacted via SWBH switchboard on 0121 507 1831, by healthcare professionals.
