Celebrating 10 years of Palliative Care in Sandwell

We were honoured to take part in Connected Palliative Care’s 10-year anniversary celebration on the 28th April.

It was a valuable opportunity to come together with partners and reflect on the achievements of the past decade.

It was a pleasure to hear Sir David Nicholson, KCB CBE, open the event – his recognition of how the Connected Palliative Care (CPC) partnership has evolved into a first-class end-of-life service for the people of Sandwell was well deserved. It was also inspiring to hear from Anna Lock, Palliative Medicine Consultant, whose passion and pride in the service shone through.

At Crossroads, supporting individuals with palliative care needs to remain in their own homes for as long as possible remains a key priority. Through our partnership with CPC, we have supported 1,184 service users (April 2016 to present) during the end-of-life period.

Through the Palliative Hub, Crossroads care support workers deliver a 24/7 service for individuals living with a terminal condition as part of the Urgent Response team. In addition, our staff provide essential respite support, enabling family carers to take a break from their responsibilities, recharge, and sustain their ability to cope during what is a very challenging time.

We look forward to continuing to strengthen and develop this valued partnership in the years ahead.
